Important Documents
In order to enroll at Lake Shore Baptist Children's Center, we must have a completed enrollment application, a $40 registration fee, the required medical form signed by your child's doctor, a copy of your child's up-to-date shot records, and the first month's tuition. Please call us at 254-772-6459 to check availability before filling out the application. The enrollment application includes both the required medical form and the get acquainted form. If you need an extra copy of either, you may click on the name of the form you're missing.
